BS Program Requirements
- Scanned copy of Birth Certificate, PSA issued
- Scanned copy of Recent Passport size colored ID pictures (white background)
- Scanned copy of Certificates of Good Moral character
- Scanned copy of Scholastic Records from the Office of the Registrar for Admission purposes only or Scanned copy of Grade 11 and Grade 12 report card (grades must have a numerical equivalent) - Grade 12 card with 1st and/or 2nd quarter grades.

Doctor of Medicine Program Requirements
- Scanned copy of Birth Certificate, PSA issued
- Scanned copy of Certificate of General Weighted Average (GWA) from Dean or Registrar
- Scanned copy of the Transcript of Records (TOR) or Scholastic Records from college last attended
- Scanned copy of College Diploma or Certificate of Candidacy for Graduation
- Scanned copy of NMAT Result
- Scanned copy of 2 pcs recent passport colored size ID pictures (white background)
- Scanned copy of Certificates of Good Moral Character or Recommendation Letter from 2 former professors
* Scanned copy of Receipt of Certificate of Eligibility is not required for admission requirements. Receipt of Certificate of Eligibility is only required for enrollment requirements.

Master in Nursing Program Requirements
- Bachelor's Degree in Nursing
- Atleast 1 year of experience as a professional nurse in a hospital/community or public/private health agency
- Scanned copy of the Transcript of Records (TOR) or Scholastic Records from college last attended
- At least 1 year of experience as a professional nurse in a hospital/community or public/private health agency
- Scanned copy of the two (2) recommendations from employer/s or school
- Scanned copy of College Diploma
- Scanned copy of 4 passport-size photos (in white background)
- Scanned copy of nursing license (PRC);
- Scanned copy of Birth Certificate, PSA issued
- Scanned copy of marriage contract (if married);
