CHED Scholarship Program for Future Medical Technologists and Pharmacists

Standards and Guidelines in the Application for the Program

Section 6. An applicant must comply with the set of criteria to qualify for the scholarship program.
Section 7. Qualification Requirements - The applicant for MTP-SP must possess the following qualifications:
7.1 Filipino citizen residing in the Philippines;
7.2 Senior high school graduate from the Science, Technology, Engineering, and Mathematics (STEM) strand or students in the higher year levels of Medical Technology/Medical Laboratory Science or Pharmacy Programs with a general weighted average (GWA) of at least 80% or its equivalent;
7.3 For incoming students, passed the entrance examinations and complied with other related requirements for admission into a bachelor degree of Medical Technology/Medical Laboratory Science or Pharmacy in the identified participating SUCs or PHEls where the scholar intends to enroll;
7.4 For continuing students, as long as they have complied with the academic requirements in the latest term/semester without any failed subjects preceding their scholarship application; and 
7.5 Combined annual gross income of parent/s, guardian which does not exceed Four Hundred Thousand Pesos (Php 400,000.00), or in cases where the income exceeds Php 400,000.00 an applicant must present a written certification or medical findings of illness of a family member, or school certifications of two or more dependents enrolled in college.

Documentary Requirements

Section 8. Documentary Requirements - The minimum documentary
requirements required at the time of application to the program are as follows:
8.1 Photocopy of Birth certificate issued by the Local Civil Registry or Philippine Statistics Authority (PSA);
8.2 Duly certified true copy of Form 138 for incoming first-year students eligible for college or duly certified true copy of grades for the latest term/semester attended for applicants with earned units in college;
8.3 The student-applicants shall submit anyone (1) of the following documents
8.3.1 Latest Income Tax Return (ITR) of parent/s or guardian;
8.3.2 Certificate of Tax Exemption/Non-Filer from the Bureau of Internal
Revenue (BIR) with Certificate of Low Income or Indigence from
their Barangay;
8.3.3 Case Study Report from City/Municipal Social Welfare and
Development Office; or
8.3.4 Latest copy of contract or proof of income may be considered for
children of Overseas Filipino Workers (OFW) and seafarers.
8.4 Other Documentary Requirements, if applicable: those applicants belonging to IPs, residing in GIDAs or areas with IP communities, and belonging to the special group of persons shall submit certifications and/or Identification Cards (IDs) issued by the appropriate offices or agencies.
Section 9. Application Procedures - The CHED/CHED Regional Office (CHEDRO) and the identified participating SUCs or PHEls, in coordination with other stakeholders, shall advocate and disseminate information on the program, its application and selection process, and the general provisions, through a call for scholarship application together with the list of identified participating SUCs or PHEls which shall be issued every academic year (AY), subject to availability of funds.
9.1 Interested applicants shall comply with the qualification requirements stated i n Section 7 of these guidelines and shall submit the application through the identified participating SUCs or PHEls.
9.2 The SUCs/PHEls shall endorse the list of scholarship applicants to the concerned CHEDRO together with the following:
9.2.1 Accomplished application form (Annex "A") with complete supporting documents; and
9.2.1 Notarized Scholar's Commitment to Render Return Service Obligation (Annex "B").

The CHEDRO shall:

9.3.1 evaluate the applications and ranks the qualified applicants;
9.3.2 inform the applicants of the status of their application;
9.3.3 send the individual Notice of Award (NOA) (Annex "C") to the qualified applicant; and
9.3.4 notify the applicant who did not qualify through email, SMS, or letter.
 Applicant accepts the NOA by affixing his/her signature on the return slip and submits the same to the concerned CHEDRO through printed or electronic form within seven (7) working days from date of receipt of the NOA;
 9.3.5 If the applicant waives the NOA, he/she shall write the word "Waived" on the return slip and affix his/her signature thereto and return the same or communicate his/her waiver in writing to the concerned CHEDRO through printed or electronic form, within the same period prescribed in Section 9.3 hereof; and
9.3.6 A Scholarship Contract (Annex "D") shall be executed among the concerned CHEDRO and approved/accepted applicant
