Master in Nursing Program Requirements

• Bachelor's Degree in Nursing
• At least 1 year of experience as a professional nurse in a hospital/community or public/private health agency
• Scanned copy of the Transcript of Records (TOR) or Scholastic Records from college last attended
• At least 1 year of experience as a professional nurse in a hospital/community or public/private health agency
• Scanned copy of the two (2) recommendations from employer/s or school
• Scanned copy of College Diploma
• Scanned copy of 4 passport-size photos (in white background)
• Scanned copy of nursing license (PRC);
• Scanned copy of Birth Certificate, PSA issued
• Scanned copy of marriage contract (if married);

For Foreign Students

• Scanned copy of birth certificate authenticated with Apostille Stamped;
• Scanned copy of passport;
• Scanned copy of I-card;
• Scanned copy of the student's visa;
• Scanned copy of Transcript of Records by the Philippine Consulate authenticated with Apostille Stamp;
• Scanned copy of the Diploma with the seal of the University and the signature of the registrar in ink. If written in another language, must be authenticated by the Philippine Consulate/Embassy with an Apostille Stamp;
• Curriculum for BS Nursing; and
